Eastchester Senior Programs and Services provides a continuum of care for older adults. By offering a range of services we assist senior citizens to remain in the community and to live independently. Our services and programs are designed to meet the changing needs of older adults and to enhance the quality of their lives.

Programs include medical and shopping transportation, case management, long term care planning, light housekeeping, home delivered meals, small home repairs, Medicare and entitlement counseling, bed and bath program, and Senior Centers at Lake Isle, Garth Road and Sundays at Concordia College.

RoccoSENIOR CENTERS

The Center at Lake Isle, 660 White Plains Road, Eastchester. Located at Lake Isle Country Club, our premier center offers an interactive environment for socializing, recreation, education, culture and entertainment. Programs range from wellness and exercise, to trips and live performances. Lunch is served daily. Transportation is available to residents of Eastchester, Tuckahoe and Bronxville. Garth Road Center at 235 Garth Road, Scarsdale. This neighborhood center is a meeting place for residents of Garth Road and the surrounding area in the Town of Eastchester. The center is open on Wednesday and Friday afternoons. Activities are tailored to the interests of participants and include an array of recreational, educational and entertainment programs. Lunch is available on Wednesdays. Call 771-3340 for information.

Sundays at Concordia, at 171 White Plains Road, Bronxville. Enjoy live musical entertainment and lunch at this unique intergenerational program. Conveniently located at Concordia College studing dining hall and scheduled on Sunday afternoons. Transportation is available to residents of Eastchester, Tuckahoe and Bronxville. DESCRIPTION OF PROGRAMS AND SERVICES

Shopping, Chores & More: A program that helps senior residents to maintain their independence by providing transportation and assistance with weekly grocery shopping and other errands. Transportation is available for trips to the supermarket, bank, pharmacy, beauty parlor, library and local shopping mall. There is a suggested contribution for this service. Available on Wednesdays, Thursdays and Fridays by reservation. Call 771-3340 for more information.

Transportation to Medical Appointments: Curb-to-Curb service for senior residents to medical appointments locally and in surrounding towns. There is a suggested contribution depending upon destination. Available on Monday - Friday by reservation. Call 771-3340 for more information.

Medical Escort Service: For seniors who need special assistance to and from medical appointments. Call 771-3340 for more information.

Light Housecleaning: This service meets the growing need of affordable housecleaning for frail or disabled seniors in our community. Light Housecleaning can be provided every two, three or four weeks. There is a co-payment for this service. Call 771-3340 for more information.

Home Delivered Meals: For seniors who are frail, isolated and homebound, a hot meal can be delivered Monday through Friday. A voluntary contribution is suggested. Call 771-3340 for more information.

Care Management and Care Planning: Our professional staff provides care management services. Home visits are made to assess needs and to assist clients and families to develop long-term care plans. Case Managers work directly with the client and their family to implement and monitor the plan through regular home visits and telephone follow-up. Support, guidance, information and referrals are provided to clients, caregivers and family members. Call 771-3340 for more information.

Medicare Counseling and Benefits Assistance: This service includes Medicare counseling, benefits assessment, and assistance choosing health insurance plans. Professional staff are available by appointment. Call 771-3340 for more information or to schedule an appointment.

Bed & Bath: This program is available to frail, at-risk elderly who require assistance with bathing and changing bed linens. The service is provided in collaboration with Family Services of Westchester. Call 771-3340 for more information.

File of Life: is a card that contains vital personal medical information. It is kept on the refrigerator in a bright red magnetic pouch, readily accessible to an Emergency Medical Team when they arrive on an emergency call. There is also a compact version that can be carried in a purse or wallet. Seniors can obtain a File of Life by calling or visiting our office. Call 771-3340 for more information.

Share the Fare Taxi Discount Program: Taxi tickets may be used by senior residents for trips within the Town of Eastchester (including the Villages of Tuckahoe and Bronxville). Taxi coupon books can be purchased at Eastchester Town Hall. Call 771-3340 for more information.

Project Lightbulb: In cooperation with the Eastchester Fire Department, this service is offered to seniors who need assistance with small home repairs. Call 771-3340 for more information.

Volunteer Opportunities: Find out how you can make a meaningful contribution to your community.

Lend a hand by delivering meals to your elderly homebound neighbors in Eastchester, Tuckahoe and Bronxville.
Help out at our intergenerational Sunday afternoon program at Concordia College.
Help senior citizens who need assistance clearing their walks and driveways of snow and ice.

Older Drivers Information
The Westchester County Older Driver Family Assistance program provides information and referrals to driving safety programs, in-car evaluations and rehabilitation. Staff is available to discuss individual concerns and offer advice on how to talk with a senior driver who may be at risk. In the event the driver must give up their license, information is available on alternative transportation and other support services. Call 771-3340 for more information.

Home Care Services When a Hospitalized Senior is Ready to Return Home
Always speak to a hospital discharge planner before the patient is discharged to inquire about services that may be available and eligibility requirements.
